Severity accounts for safeguards drawn on the P&ID: relief devices, interlocks, alarms, check valves. Rows where no safeguard is visible carry a higher residual score and a note.

Most of a HAZOP session's first day is spent applying the guideword matrix from a blank worksheet. Walking each node, crossing it against No/None, More, Less, As Well As, Part Of, Reverse, Other Than for every applicable parameter, listing credible deviations, scoring consequences. Work that is systematic and repeatable. HAZOP Assistant does that part: walk every drawn node, apply the full guideword × parameter matrix, pre-fill every cell, score four severity categories, anchor the safeguards to tags.

What it does not do: chair the session, interpret operating context, weigh trade-offs, or sign off on the action register. That stays with the team in the room.

6–12 nodes per drawing, depending on complexity. Each major piece of equipment — compressor train, heat exchanger, separator, reactor, column, header — is treated as a node. The node count and list are reported at the top of the output.

No/None, More, Less, As Well As, Part Of, Reverse, Other Than — the full IEC 61882 guideword set. Custom guideword sets or project-specific additions can be configured per study.

Flow, pressure, temperature, level, composition, reaction, utilities — plus any process-specific parameters added during configuration. Guideword × parameter combinations that are physically implausible for the node type are flagged as N/A, not silently dropped.

Safety, environment, asset, and production — each scored independently. A deviation that is low safety risk but high production risk gets the right score in each column, allowing the study leader to filter by audience.

The demo uses a 4-category 3-level matrix (people / asset / environment / reputation × high / medium / low). For your study, you upload your corporate risk matrix and the tool scores against it. Severity anchors, likelihood definitions, and risk-rank colouring all follow your matrix, not ours.

A node × guideword matrix coloured by deviation count. High-activity cells show which node–guideword combinations generated the most rows. Low-confidence cells are flagged. The study leader uses it to decide where to spend session time and where the tool's output can stand as-is.
